Always try to grab a business card on your way out.

In some types of shops you can actually get the employee to write a few notes for you on a note pad. After all, normal customers often do this.
As a normal customer I would ask for the employee's name, and actually write it down myself in front of them on the note or on a brochure.

For promotional activity in-store, always take the time to browse, even if it's not in the actual scenario. This will provide you with an opportunity to look around and recall posters and advertising which you might need in your report.

Always try to put something new in there. Some additional information not dealt with in the questionnaire. If you are stuck for things to write, just state how you felt about the shop.

Some stores have the name of the shop assistant on the receipt. Learn the ones that do. Always get a receipt when it is available.

Sometimes the outside of the store can be photographed on your moblie.

Look carefully at the store before you leave your car and note outside questions which are asked, such as the cleaniliness of the car park, the advertisement of opening times or the window displays.
